A conference entitled 'The appropriate use of investigations and treatment in clinical practice' was held at the Royal College of Physicians on 28 November 1988. We publish here the papers given at the meeting in order to promote a more critical evaluation of diagnostic and therapeutic procedures. Many textbooks record indications for investigations. There is a danger that an unthinking houseman will use such lists as shopping lists of tests that ought to be carried out in order that a patient may be properly 'worked up'. It is the responsibility of all those who train young doctors to make them think in terms of what tests are appropriate, and cost-effective. A number of examples are given in this collection of papers to show that clinical information may be a more powerful predictor of outcome than any investigation. Some clinicians think that an audit of investigations and procedures, if introduced, will largely be a cost-cutting exercise. Although undoubtedly there is enormous scope for saving money by avoiding inappropriate investigations and procedures, the paper by John Hampton and colleagues shows how a carefully constructed audit may reveal large numbers of patients who have been inappropriately under-investigated. The papers published here illustrate a variety of ways of measuring 'appropriateness'. Mark Chassin, for example, reports the experience of workers at the RAND Corporation in establishing appropriateness by the consensus judgement of expert panels. Others, including Christopher Mallinson writing about appropriate investigation in gastroenterology, and myself on neurological disorders, record that appropriateness of a test can best be judged by its success in predicting a defined outcome. Jack Wennberg at the meeting stressed the necessity of informing the patient about possible outcomes after a procedure and of including the patient fully in the decision about whether to undergo it.
